Aja Health and Wellness Ltd. ("AJA") respects the privacy of all of its users and is committed to protecting and respecting such privacy in accordance with this Privacy Policy. This Privacy Policy explains AJA's collection and processing of your personally identifiable information collected in connection with your activities on or related to all AJA Web Sites referencing this Privacy Policy (the “AJA Sites”) and all services made available on the AJA Sites (collectively, unless noted otherwise, the “Services”). This Privacy Policy also explains how you can ask questions and make suggestions about our privacy practices and review and update your personal information. This Privacy Policy should be read in conjunction with our Terms of Service, available on this website.

We urge you to read this Privacy Policy carefully and to check back periodically as this Privacy Policy may change from time to time. Please note that this Privacy Policy does not apply to the practices of companies or Web Sites which are not owned or controlled by AJA.

Any reference in this policy to "you" or "your" or other similar words, shall be deemed to include all your heirs, trustees, administrators and other legal representatives as well as any dependents over which you have legal guardianship and legal authority to act on the behalf of.

Capitalized words not otherwise defined in this Privacy Policy have the meanings given to them in our Terms of Service.

"Personal Information" means information about you that specifically identifies you or, when combined with other information we have, can be used to identify you. This includes the following types of information:

  • Contact Information: your name, postal addresses, email addresses, social networking website user account names, telephone numbers, or other addresses at which you are able to receive communications;
  • Relationship Information: information you provide that enables us to determine lifestyle, interests, and activities, including location information related to your province, city, or neighborhood; areas of interest, information collected through your interactions with social networks, demographic information (e.g., birth date, age, gender).
  • Transaction Information: information you provide when you interact with us and the AJA Sites, such as email and other communications.
  • Health Information: information regarding your personal health and your healthcare number or other identification to use to facilitate payment between you and your Provider. Your express consent is provided to collect your personal health information.

Non-Personal Information” is information of an anonymous nature, such as an Internet Protocol Address (“IP Address”), the domain used to access our service, and the type and version of browser or operating system being used. Aggregate information, such as demographic statistics of our users, number of visitors and average time spent on a service, is also considered Non-Personal Information.

Our model uses online video conferencing and Internet-reliant, application-based technology.

COOKIES

From time to time when you visit a website that offers our Services, or when you click on a link that connects to our Services, information may be placed on your personal computer and/or mobile device (collectively, on the "device"). This information is commonly stored in a "cookie". Cookies are small text files stored in a web browser or cookie-enabled mobile application. COLLECTION OF PERSONAL INFORMATION

WHAT PERSONAL INFORMATION DO WE COLLECT?

We collect the following information as required in order to provide services to you, confirm your identity, diagnose and provide treatment for your health condition, and to administer the account for the services such as billing, insurance, or other administration. We also create an account with your Personal Information that allows us to serve you better as a repeat user.

In particular, some of the personal information that we collect about you in order to provide the Services, includes, without limitation:

  • email address, physical and contact information;
  • transactional information based on your activities on AJA Sites;
  • billing and other information you may provide;
  • personal information regarding your health you may provide;
  • community discussions, chats, dispute resolution, correspondence through AJA Sites, and correspondence sent to us;
  • other information from your interaction with AJA Sites, Services, content and advertising, including computer and connection information, statistics on page views, traffic to and from the sites, ad data, Internet Protocol ("IP") address and standard web log information; and
  • information from other companies, such as demographic and navigation data.

LIMITS ON COLLECTION

We collect only the Personal Information that is required by us and Provider to provide the services you have requested. We do not collect any other information, or allow information to be used for other purposes without your express (written or verbal) consent – except where required by law.

We use and disclose your Personal Information in the following circumstances:

  • to schedule a visit with a Provider and to send any appropriate records to the Provider;
  • storing clinical data electronically;
  • for administration of billing;
  • to other healthcare practitioners involved in your care, if applicable;
  • as required by law, such as to comply with a subpoena, or similar legal process;
  • when we believe in good faith that disclosure is necessary to protect your health, your safety or the safety of others, investigate fraud, or respond to a provincial government request;
  • if GOeVisit and/or AJA is involved in a merger, acquisition, or sale of all or a portion of its assets, you will be notified via email and/or a prominent notice on our web site of any change in ownership or uses of your personal information, as well as any choices you may have regarding your personal information;
  • to any other third party with your prior consent to do so.

We will disclose your Personal Information where such disclosure is required by law, such as to comply with a subpoena or court order or records.

CONSENT

Your acceptance of this Privacy Policy/use of our Services constitutes your express consent to use your Personal Information to refer you to a Provider, to send your records and schedule the visit with the Provider, to create a profile for you, to facilitate billing, and to send your clinical data to electronic medical records systems.

Any individual purporting to make decisions on your behalf may be asked to validate his/her authority to do so.

We will not disclose your Personal Information to third parties unless you have specially given us consent to disclose your Personal Information for a designated purpose.

AJA will use Non-Personal Information collected through the provision of services to: to develop data on patient health and patient care, industry and market analysis, demographic profiling, marketing and advertising, and other business purposes.

You can withdraw your consent to have your Personal Information stored with AJA and to have your Personal Information disclosed with applicable third parties previously consented to by: emailing clientsupport@goevisit.com or calling 1-866-318-4748. We retain Personal Information from closed accounts to comply with law, prevent fraud, resolve disputes, troubleshoot problems, assist with any investigations, enforce our Terms of Service and take other actions otherwise permitted by law.

ACCESSING YOUR PERSONAL INFORMATION

On written request, you will be informed of the existence, use and disclosure of your Personal Information that is under our control, and may be given access to that Personal Information as required and permitted by law. You are entitled to challenge the accuracy and completeness of that Personal Information and request that it be amended, if appropriate. If you would like a physical or electronic copy of your records, a reasonable fee will be charged.

PROTECTING YOUR PERSONAL INFORMATION

AJA is not responsible for any third parties, their use of AJA, and how they retain or secure your Personal Information.

We are continuously implementing and updating administrative, technical, and physical security measures to help protect your Personal Information against unauthorized access, destruction or alteration. AJA operates works with security experts and industry professionals to make sure your health data is protected. However, no method of transmission over the Internet, and no method of storing electronic information, can be 100% secure. So, we cannot guarantee the absolute security of your transmissions to us and of your Personal Information that we store.

You can help protect the security of your Personal Information as well. For instance, never give out your password, since this is what is used to access your account on the AJA Sites, and ensure updated security software is installed on your computer. Also remember to log out of your account and close your browser window when you finish using the AJA Sites, so that other people using the same computer won't have access to your information.

We instruct our employees with access to your Personal Information that it is to be used only in adherence with the terms of this Privacy Statement. However, we are not responsible for Providers' activities or their use of your Personal Information. We will not otherwise disclose your Personal Information without a subpoena, court order or substantially similar legal procedure, except when we believe in good faith that the disclosure of information is necessary to prevent imminent physical harm or financial loss or to report suspected illegal activity.
